Complete the following forms and return them to the Society

(Attn: EVOLVE Intern Programme, Pharmaceutical Society of New Zealand, PO Box 11640 , WELLINGTON 6142):

• Year 2012 Intern Placements

• Approval as an Intern Training Site

• Preceptor Approval

• Workplace Assessor Training form

The date for the next Workplace Assessor Training will be 26 July 2011. This will be held in Wellington at the Abel Tasman Hotel.

Letters will be sent out to advise those that will need to attend.
