Complete the following forms and return them to the Society

(Attn: EVOLVE Intern Programme, Pharmaceutical Society of New Zealand, PO Box 11640 , WELLINGTON 6142):

• Year 2011 Intern Placements

• Approval as an Intern Training Site

• Preceptor Approval

• Workplace Assessor Training form

The dates for the next Workplace Assessor Training will be July 2010 (Date to be advised). This will be held in Wellington at Pharmacy House.

Letters will be sent out to advise those that will need to attend.
